Unlike many disasters, aviation accidents are almost entirely the result of human choices—often made by well-trained people doing everything they reasonably can to keep us safe. So how do these rare but dramatic failures still happen?
In a brisk 45 minutes, Lt Col (ret) Ryan Hart will walk us through real-world crash investigations to uncover how small errors, miscommunications, and system failures can compound into catastrophe—and what can be done to stop that chain before it ends in tragedy. Along the way, you’ll gain a clearer understanding of why flying is still remarkably safe, and what aviation can teach every industry about building resilient systems.
Ryan is a retired Air Force pilot and Chief of Safety, a trained aviation mishap investigator, and now a commercial airline pilot. He also happens to love talking about airplane crashes.
