Clinicians regularly encounter patient presentations that extend beyond the relatively simple question of whether a tooth requires restoration. The more difficult—and often more consequential—decision is whether to retain and restore a compromised tooth or remove it and replace it with an implant.
During this presentation, Dr. Spear will examine the factors that influence these complex treatment decisions, with particular attention to the potential long-term advantages of retaining and restoring natural teeth, especially in younger patients. Through long-term clinical examples and a review of the supporting literature, attendees will gain a practical framework for evaluating teeth that may initially appear questionable or hopeless.
Learning Objectives
By the end of this program, attendees will be able to:
- Apply an algorithm of key questions when determining whether to retain and restore or remove and replace a compromised tooth.
- Discuss what the literature reveals about long-term success rates for different clinical conditions that may appear questionable or hopeless.
- Evaluate how a patient’s age can affect treatment decisions and explore the concept of “management versus cure.”
- Look beyond the immediate treatment decision to consider “what comes next” and how future treatment needs may influence what should be done today.

About Dr. Frank Spear
Dr. Frank Spear is the founder of Spear Education in Scottsdale, Arizona, and remains one of the world’s most respected educators in esthetic and restorative dentistry.
He earned his DDS in 1979 and his MSD in Periodontal Prosthodontics in 1982 from the University of Washington. Throughout his career, Dr. Spear has received numerous prestigious awards recognizing his contributions to dental education, including honours from the American Academy of Esthetic Dentistry, the American Academy of Cosmetic Dentistry and the Seattle Study Club.
Dr. Spear is also a member of several leading dental organizations, including the American Academy of Esthetic Dentistry, the American Academy of Restorative Dentistry and the American College of Prosthodontists.
