Join Healthwatch Wandsworth, Ruils, St George’s and other local organisations for a special Healthwatch Wandsworth Assembly exploring how we can understand and improve hospital discharge.
We want to hear directly from patients and carers about their experiences, what worked well, what could have been better, and what needs to change further to the implementation of the NHS England Carers and Hospital Discharge Toolkit.
We are keen to listen to insights from carers and patients and to identify further areas for improvement.
