Positions: Direct Care Counselors and Habilitation Assistants

Direct Care Counselors

Locations: All 5 boroughs (Queens has the most vacancies at this time)

Pay: $20- $20.37/hour

Shifts: 6:00AM-2:15PM, 2:00PM-10:15PM, 10:00PM-6:15AM

Requirements:

• Must be able to understand and communicate in English.
• Experience working with individuals with developmental disabilities preferred.
• Must demonstrate satisfactory completion of CP Unlimited’s orientation training program.
• Must demonstrate satisfactory completion of the Office for People with Developmental Disabilities (OPWDD) Approved Medic*tion Administration Personnel (AMAP) course and Annual Medic*tion Re-Certification.
• Must be able to perform basic data entry on a personal computer (PC).
• If a person supported requires specialized treatments/medications /procedures (ex.: g-tube feeding, insulin administration) must demonstrate satisfactory completion of a specialized course pertinent to the person’s needs and obtain required certification and annual re-certification in order to remain working in a designated site.
• Must be willing to work in multiple agency locations (“float”) according to agency floating protocols.
• Depending on residence a NYS driver’s license may be required in order to be assigned to work in that location.
• Must be able to work overtime as needed.
• Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.

Habilitation Assistant

Location: Long Island City (Queens) Day Program

Pay: $20.37/hour

Shift: 8:30AM-4:30PM (Monday-Friday)

Requirements:

• H.S. Diploma
• Minimum 1-year experience in related field.
• Successful completion of AMAP, CPR and other required training including PrecisionCare, and basic MS.Word preferred
• NYS Driver’s License preferred
• Must be willing to work outdoors for community based activities
• Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
