15 October PLT Workshop: Accelerating Outpatient Redesign and Care Closer to Home This workshop will bring together clinical colleagues from BHT, GP Practices, PCNs and FedBucks to explore how we can move further and faster with outpatient redesign and the shift of care closer to home. The session will connect this work with the wider neighbourhood health agenda, supporting delivery across organisational boundaries and strengthening collaboration across the system.The focus will be on creating a practical, action-oriented discussion that moves beyond identifying challenges and towards designing and delivering solutions together. Building on conversations through the Clinical Interface Group and Neighbourhood Programme Group, the workshop will consider how we can overcome organisational silos, challenge "as is" thinking and accelerate change in areas where there is shared ambition for further progress.
