This is the registration form for the Community Emergency Response Team (CERT) Basic Academy in the City of Ontario.About this Event
The class is FREE and open to anyone interested in being better prepared for emergencies or disasters. Please complete one registration form per individual (NOT HOUSEHOLD). Minors 16 or older will be required to obtain parent/guardian approval prior to taking the course. You must complete all of the information including supplemental fields for your registration to be accepted.
The class will be taught in three parts:
Thursday, October 15th at 6:00pm-9:00pm
Saturday, October 17th at 8:00am-5:00pm
Sunday, October 18th at 8:00am-5:00pm
Participants must be present for the full duration of the class.
