The MyHealth Programme is committed to ensuring that the residents of Hillingdon can access high quality, evidence-based care in a setting appropriate to their needs by transforming the way care is delivered. We do this by co-producing the programme with our service users and carers, providers, the local authority and community and voluntary sector groups.
This is a 2 hour session for people who have been diagnosed by their GP having type 2 diabetes
During the session we will cover
- Steps to help you manage your Diabetes
- Diet and Nutrition advice
- Advice on avoiding Diabetes complications
- Medic*tion and checks you should be having
- Questions and Answer session
All information presented in the workshop is from a fully qualified health professional this can be either a GP from the local practice or a clinical pharmacist.
We encourage you to ask any questions relating to diabetes as the more you ask the more you can learn.
