TCPR will once again be hosting a wellness clinic on Sunday 11/8/26 at our office in Maplewood (1983 Sloan Pl Suite 16 Maplewood, MN 55117)Please complete this form to indicate your interest in attending the wellness clinic. A second form will be sent ahead of the event to the email address provided to collect all medical and contact information needed and to reserve your spot at the event. Both forms will need to be completed to ensure your pet is seen. Exams will be completed on a first come, first served basis - you will not receive an appointment time, but you may be asked to stagger arrival times. Please note that spots may be limited if interest exceeds capacity and there will likely be a wait once checked in. All pets confirmed and checked in between 11am to 1pm will be seen.
