WHEN COMPLETING THE BOOKING FORM PLEASE ANSWER ALL QUESTIONS FULLY. YOUR BOOKING IS NOT CONFIRMED WITH LIMBPOWER UNTIL YOU HAVE COMPLETED ALL OF THE QUESTIONS ON THE FORM, PARTICULARLY THE MEDICAL INFORMATION.
Please note: If you have been admitted to hospital in the 12 months prior to the event, we will send you an additional medical form which will need to be signed by your doctor to confirm you are fit to participate. We ask that these are returned as soon as possible.
