Join Wise & Well for Hospital Discharge Planning, an educational session focused on helping older adults and caregivers navigate the transition home safely and confidently after a hospital stay.
Participants will learn:
• common discharge mistakes to avoid
• questions to ask before leaving the hospital
• how to prepare the home for recovery
• Medic*tion and follow-up planning tips
• warning signs that require additional support
• strategies that can help reduce fall risk and readmission
This session is part of Keswick’s Falls Prevention Week programming and is designed to provide practical guidance for safer transitions and stronger recovery outcomes.
