What is the Program of All-Inclusive Care for the Elderly (PACE)?
PACE is a coordinated care model where an interdisciplinary team provides medical care, long-term services, and social supports to help older adults live independently in their communities
ECM and PACE can help skilled nursing facilities connect eligible residents with the support they need
