An introduction to creating and utilizing advance directives, choosing a healthcare agent, and understanding ACP forms.
About this Event

Advance care planning is the process of thinking about, discussing, and documenting preferences for future medical care when a person is unable to speak for themselves.

Participants will explore the purpose and components of commonly used advance care planning documents, including state-specific advance directives for health care, choosing a healthcare agent to speak for you when you cannot speak for yourself, Do Not Resuscitate (DNR) orders, Physician Orders for Life Sustaining Treatment (POLST) orders, and designating one’s body for organ donation.

Attendees will gain a comprehensive understanding of the Virginia Health Care Decisions Act, learn how to create and utilize an advance directive, and engage in discussion with a curious group of like-minded peers.
