- Fall 2025 initiates: will be entered by their Province MTA representative, but please check with the Province MTA Coordinators for special instructions.
- Spring 2025 initiates: *IF* your chapter collected CRWLC fees at the time of your initiation, those funds still must be paid to the province.
If you have verifed that this applies to you, do not attempt to complete this form without payment. Instead, either a) complete the form with payment, then seek reimbursement from your chapter, or b) provide your information, including meal preference, to your chapter's Keeper of Exchequer so that he can complete your registration.
