About this Event

From Referral to Results:

How Multidisciplinary Teams Improve Outcomes in Complex Rehabilitation Cases
Seminar hosted by Manchester Occupational Therapy Services
Proudly sponsored by Independence Mobility

“Getting the right clinicians working together at the right time.”
This is the single biggest challenge we hear from case managers coordinating complex rehabilitation cases.

When communication between professionals is fragmented or roles are unclear, progress can slow and case management becomes significantly more demanding.

This seminar is designed specifically for case managers working in complex rehabilitation and focuses on practical insight that can be applied directly to real cases.

Understanding how a well coordinated multidisciplinary team operates can make a substantial difference to how efficiently complex cases move forward.

Hosted by , this half day seminar will explore how effective collaboration between occupational therapy, physiotherapy, psychology and mobility specialists supports smoother case management and better outcomes for clients.

Using real client case examples, our speakers will walk through the rehabilitation journey from initial referral through intervention and progress, demonstrating how coordinated multidisciplinary rehabilitation can reduce delays, remove barriers to progress and support meaningful functional gains.

Attendees will gain insight into how professionals within complex rehabilitation and neurological rehabilitation work together, how communication between clinicians and case managers can be streamlined, and how timely clinical decision making can support more effective rehabilitation planning.

The event is designed for professionals working across Stockport, Greater Manchester and East Cheshire, and also provides an opportunity to connect with others working in case management and multidisciplinary rehabilitation across the region.
