Duffine Family Cardiac Transition Education Day: The Beat Goes On

Transitioning from pediatric CHD care to adult CHD care can feel like entering a whole new world — new doctors, new systems, new responsibilities, and new questions. This day is designed to help young adults with congenital heart disease build confidence, learn practical tools, and connect with others who understand the transition experience.

Attendees will have the opportunity to meet CHD care team members, hear patient stories, participate in interactive sessions, and learn more about navigating adult care.

Topics will include mental health, college, exercise, insurance navigation, MyChart, pregnancy and contraception, and real-life conversations with CHD patients and providers.

The day will also include giveaways, a photo booth, interactive activities, and time to connect with other young adults, families, caregivers, and care team members.

Light breakfast and lunch will be served.
