Chapman Consulting facilitates two Collaborative Planning and Implementation (CPI) workgroups on behalf of the Department of Health Care Services. The CPI collaboratives bring together local Enhanced Care Management (ECM) and Community Supports stakeholders to work together to identify, discuss, and resolve topical implementation issues, improve outcomes, and to shape planning and implementation activities.
The San Francisco County Collaborative includes local stakeholders representing San Francisco City and County, providers, clinics, and community-based organizations.
