Meet Irene: How Community Care Delivers Realistic Medicine at Every Stage
This session provides an opportunity to explore four services that demonstrate the core principles of Realistic Medicine, including person‑centred care, shared decision making, and a focus on what matters most to individuals. By using one of NHS Tayside’s personas, Irene, as a common reference point, the session places each service within a realistic patient journey. This approach supports a clearer understanding of how services can work together to deliver care that is personalised, coordinated, and value‑based, while minimising harm and unwarranted variation.
