VOLUNTARY PARTICIPATION AND ASSUMPTION OF RISK

I understand that participation in dance fitness classes involves physical activity, including but not limited to dance, cardio exercise, and strength movements, which may involve risks such as falls, collisions, overexertion, muscle strain, or other injuries.

I acknowledge that I am voluntarily participating in activities offered by Charlotte Lister Dance Joy LLC and that I assume all risks associated with participation, whether known or unknown.

I confirm that I am physically able to participate and will monitor my own condition. If I experience pain, discomfort, or unusual symptoms, I will stop immediately.

RELEASE OF LIABILITY

In consideration for being allowed to participate, I hereby release, waive, and discharge Charlotte Lister Dance Joy LLC, its owner, instructors, contractors, and representatives from any and all liability for injury, illness, death, or property damage arising out of or related to my participation, including any claims arising from negligence.

INDEMNIFICATION

I agree to indemnify and hold harmless Charlotte Lister Dance Joy LLC and its representatives from any claims, demands, or causes of action arising from my participation in the class.

MEDICAL ACKNOWLEDGMENT

I understand that this activity is not medical advice and that I should consult a physician before participating if I have any concerns about my health.

I grant permission for my image, video, or likeness to be used for promotional purposes, including social media and marketing, without compensation.

By registering for this class, I acknowledge that I have read and understand this waiver and voluntarily agree to its terms.

This agreement shall be governed by the laws of the State of California
