By attending this event, you recognize, understand, and adhere to the following:

You recognize and understand the risks of physical injury inherent to dance and dance training and fully assume those risks. You hereby release G.R.I.T. Line Dancers, Inc, its affiliates, members, dance studios, event sponsors, employees and dance teachers from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, free or paid line dance lessons, public events, or performances. You agree to indemnify, defend, and hold harmless G.R.I.T. Line Dancers, Inc, its affiliates, members, dance studios, event sponsors, employees and dance teachers for liabilities, costs & judgments arising from acts of omissions committed by you/ your child which result in injury or damage to any person or property. You understand that images and/or videos may be used in print/online publications, websites, and social media. You release liability from all claims of publication and understand no royalty, fee, or other compensation shall become payable to you by reason of such use. Attending this event will suffice as signing for this agreement. You acknowledge and represent that you have fully informed yourself of the content of the waiver and hold harmless agreement by reading it before you sign it, and understand that you sign this document as your own free act and deed. You must be at least twenty-one (21) years of age and fully competent to sign this agreement.

Waiver

I acknowledge the contagious nature of the Coronavirus/COVID-19 that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that G.R.I.T. LINE DANCERS, INC/ELKS has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that G.R.I.T. LINE DANCERS, INC/ELKS can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others attending this event.

I voluntarily am attending this event and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending this event including mask mandate, social distancing, and temperature checked.

I attest that on the day of the event:

• I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
• I have not traveled internationally within the last 14 days.
• I have not traveled to a highly impacted area within the United States of America in the last 14 days.
• I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
• I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
• I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

If any of these statements are not true, you should not attend this event. Do note, No refunds will be given.

I hereby release and agree to hold G.R.I.T. LINE DANCERS, INC/ELKS harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act at this event, or that may otherwise arise in any way in connection with any services received from G.R.I.T. LINE DANCERS, INC/ELKS. I understand that this release discharges G.R.I.T. LINE DANCERS, INC/ELKS from any liability or claim that I, my heirs, or any personal representatives may have against them with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from G.R.I.T. LINE DANCERS, INC/ELKS. This liability waiver and release extends to G.R.I.T. LINE DANCERS, INC/ELKS together with all members, owners, partners, and employees.
