Embedding Person-Centred Care in Everyday Practice
Duration: Full day course - 9.30am arrival for 10:00 start.
Course Overview:
- Define what is meant by person-centred care and why it’s essential to quality support
- Understand the legal and regulatory framework that underpins person-centred practice, including the Care Act 2014 & Mental Capacity Act 2005
- Explore the values of dignity, choice, independence, respect, privacy, and inclusion
- Identify the key features of person-centred approaches and how they apply in day-to-day care
- Understand how to support individuals to make informed decisions about their own care and wellbeing
- Learn how to involve individuals, families, and advocates in care planning and reviews
- Gain awareness of how to assess individual needs, strengths, preferences, and outcomes
- Understand how to promote meaningful choice while balancing safety and positive risk-taking
- Explore the link between person-centred care and improved physical and emotional wellbeing
- Learn how to document and review care plans that reflect a person’s wishes and changing needs
- Understand how to work in partnership with multidisciplinary teams to deliver joined-up, personalised care
- Recognise the role of communication and active listening in person-centred planning
- Appreciate how person-centred approaches apply to a range of care areas including dementia, learning disabilities, mental health, and palliative care
