The clinical skills series is for Health Care Professionals and for care workers caring for clients either in Aged Residential Care, a General Hospital or community setting who have palliative care needs.
The education sessions provide learning which maintains and enhances participants palliative care knowledge, skills and practice.
The aim of this presentation is to:
- Recognise the value of having an ACP
- Increase understanding of ACP process
- Identifying the barriers to ACP
- Integrating ACP into clinical practice in different settings
- Understand Advanced Directive documentation
- Understand the process sharing an ACP with health providers
