This form is for children who wish to serve on Kidz First Sunday (23rd Nov 2025) at Jesus House in various ministries.

Please do not complete the form for children already taking part in the Kidz First Choir or Dance or Drama groups.
Children who are serving will be required to arrive in church by 8:30am on the day.
Some departments will require an induction on an earlier date and some may also require a dress code for the day, this will be communicated to you once your child’s department has been confirmed.
