
About this Event
The Victorian Cancer Survivorship Community of Practice forum brings together health professionals with an interest in cancer survivorship and the implementation of new models of care and provides an opportunity to discuss key aspects of cancer survivorship.
This free forum is open to those working with cancer survivors from all sectors including hospital, community or non-government organisations, researchers and policymakers.
This is a joint initiative between the ACSC, Victorian Integrated Cancer Services (VICS) and the Department of Health.

Day: Wednesday 29th October 2025
Time: 9.30am registration, commencing at 10.00am - 2.30pm
Location: Level 13, 305 Grattan St, Melbourne, Victorian Comprehensive Cancer Centre (VCCC), Peter MacCallum Cancer Centre building. Access Level 13 via the ‘Research Lifts’, immediately to the left when you enter the building from Grattan Street.
The forum will include:
- Welcome address and ACSC updates
- Department of Health update
- Consumer advocate story
- Presentations from the VICS Statewide Survivorship Program grant recipients
- Interactive discussion focused on:
(i) insights into practice and processes for discharging patients who are ~5 years post-treatment
(ii) practice and processes to prepare patients for end of treatment and the survivorship phase of care
