Self-Direction: Understanding the Role of the Care CoordinatorMay 28 | 5–7 PM
Self-Direction offers individuals and families more choice and flexibility in how services are used - but navigating the process can sometimes feel overwhelming. In this month’s Speaker Series, we’ll take a closer look at the role of the Care Coordinator and how they help guide individuals and families through the Self-Direction process.
Katelyn Luke-Moore from Person Centered Services will walk through how Care Coordinators work alongside members and their families to secure service authorizations, support informed decision-making, and help build strong support teams so services are used in ways that truly meet each person’s goals.
Katelyn has been with Person Centered Services for more than six years, previously serving as a Care Coordinator, Care Coordinator Ambassador, and Supervisor before stepping into her current role as Provider Relations Manager. She now works with community organizations and OPWDD agencies across the region to provide education and guidance around Care Coordination services.
