Join us to learn how you can refer your patients that have Medi-Cal and a chronic health condition for free medically tailored, nutritious meals. Roots Food Group provides chef-designed meals delivered directly to eligible patients’ homes that are fully covered by Medi-Cal insurance.
Sara Tindaro, MS, RDN is a Community Supports Dietitian at Roots Food Group that will present on this new program for patients with Partnership Medi-Cal, the eligibility criteria, and the referral process.
