The Division of Prevention Services is excited to invite you to a special event celebrating the launch of the School-Based Early Support programs. This event will bring together community providers, DOE administrators, and ACS leadership, to showcase our collective efforts in supporting families across New York City.

By registering for the School-Based Early Support Launch Event, I hereby grant the City of New York ("City"), acting by and through the Administration for Children's Services ("ACS"), and its designees:

1) The right to photograph and record my face, likeness, appearance, and voice, in whole or in part ("Image(s)"), for use in the creation, distribution, promotion, or publicity of any publication in any media including, but not limited to, digital, electronic, print, television, film, radio, ("Work(s)"); and

2) All ownership and copyright rights to the Image(s) or any resulting Work(s) which contains or features the Image(s).
