Youth Basketball is a sports activity and you are aware that accidents do occur; by your registration for this program you are acknowledging that fact and indemnify Saint Philip Church, their staff and coaches.

I grant Saint Philip AME permission to use my child’s image, likeness, and/or voice as captured in photographs, video recordings, or audio recordings. These may be used in print, digital, or social media formats for promotional, educational, or other lawful purposes. I understand that I will not receive compensation for this use.

By your registration Parents/Guardian is granting permission for your child to participate in the Saint Philip Basketball Program.
