Roseland Equity and Community-Driven Health (R.E.A.C.H.) is a collaborative committed to transforming health outcomes to the Far South Side of Chicago through equitable and compassionate care. Focused on maternal wellness, behavioral health, and whole-person support, R.E.A.C.H. brings together partners, providers, and residents to close health gaps and build a healthier future.
Roseland Equity and Community-Driven Health is a bold response to deep-rooted disparities in maternal and behavioral health outcomes on Chicago’s Far South Side.
Born out of a shared vision between healthcare leaders, community advocates, and local institutions, R.E.A.C.H. was created to close the health equity gap by meeting people where they are—with care that is compassionate, coordinated, and culturally responsive.
We recognize that too many families in the Far South Side of Chicago fall through the cracks due to fragmented systems, lack of access, and generational barriers to wellness. So we designed something different: a collaborative, neighborhood-based model that centers community voices, addresses social determinants of health, and prioritizes both prevention and follow-through.
Through strategic partnerships, on-the-ground support, and a deep commitment to justice, R.E.A.C.H. is more than a healthcare initiative —it’s a movement to transform how care is delivered and who it truly serves.
