- This play group occurs in a group setting where other families and children are present. While every effort is made to respect the privacy and confidentiality of all participants, the nature of a group environment means that personal information, including a child’s participation in speech therapy services, may be observed or inferred by others.
- Protected Health Information (PHI) will not be intentionally disclosed by the speech-language pathologist outside of what is permitted under the Health Insurance Portability and Accountability Act (HIPAA). However, incidental disclosures may occur as a result of participation in a group setting.
- By attending, you consent to participate in this group environment and acknowledge the potential for limited confidentiality. You also agree to respect the privacy of other participants and refrain from sharing any personal or identifying information about other children or families.
