Welcome to the Own Your Next Chapter: College Access and Readiness Workshop! Join us at Curry Student Center, Room 333, on January 24, 2026 at 10:00 AM for an engaging event focused on preparing for college.

After you register, we will follow up with additional information including a link to the three forms noted here for you to return to us: Parental/Guardian Affirmation Waiver and Release, Photograph, Media, and Video Authorization Release Form, and Youth Initiative Virtual Meeting/Event Participant Agreement.

Parent-Guardian Waiver and Release Form

In order to participate in the program the Parent/Guardian Waiver and Release must be signed. We kindly ask you review the form and bring it with upon arrival- if youth are being dropped off or arriving alone. Alternatively, this form can be completed on-site by a parent or guardian.

Optional- Photo Release Form

During the event we will be taking photos to capture the day. We will use these photos to promote upcoming youth programming including this workshop in the future.

If you are comfortable with your photo being taken, we kindly ask that you review the form and bring it with you upon arrival- if you are being dropped off. Alternatively, a form can be completed on-site by a parent or guardian.
