Who Should Attend?

The two day CareCoordinatorPlus course is ideal for:

  • Newly appointed or currently in-role Care Coordinators who wish to take a more professional approach to their role.
  • Those in other health and caring professions, or those with lived experience, who are considering applying for a Care Coordinator role in a Primary Care Network or voluntary sector scheme.



About the CareCoordinatorPlus Programme

The Care CoordinatorPlus Programme is PCI Accredited and fully aligned with the 2023/24 Care Coordinator Workforce Development Framework. The Programme comprises 4 half day modules, three of which take place in the same week, with the fourth Active Learning Set and Reflective Practice session taking place several weeks later, once delegates have had an opportunity to put their newly learnt skills into practise.

The intensive online programme is delivered via the Teams online video conferencing platform. It is designed to provide delegates with all the knowledge, skills and confidence needed to allow them to commence work as a Care Coordinator in a GP surgery or a community based Social Prescribing scheme.



What Will I Learn?

Whilst every member of a Personalised Care team should have the same set of basic skills, techniques and approaches needed for their role, the Care Coordinator is often asked to take on further duties specific to their role.

For example, in addition to sharing in the team’s caseload of patients, Care Coordinators working at PCN level are often asked to support other clinicians in providing the PCN's commitment to Care Homes under the Enhanced Health in Care Homes (EHCH) initiative.

Care Coordinators may also take on additional coordination roles within the PCN or Practice; they may triage referrals into the team from the PCN's Surgeries, convene and manage Multi Disciplinary Teams (MDTs) at PCN and practice level and build relationships with community groups and voluntary sector organisations. They may also take responsibility for identifying and working with cohorts of vulnerable patients, leading for example on the PCN/Practice's programmes around Anticipatory Care and Health Inequalities, and providing their patients with Personalised Care and Support Planning, a key part of the Care Coordinator role.

These additional roles will vary depending on the requirements and priorities of the PCN or Practice, but all require skill sets and knowledge over and above those used by their team colleagues.

The CareCoordinatorPlus Programme provides Care Coordinators with the essential knowledge, skills and confidence to embrace these ‘special to role’ skills and to make an immediate and positive impact on the work of the PCN or Practice.



Ongoing Support and Peer Support Networks

As with all of our courses, delegates receive access to 12 months of 24/7 telephone support following the training, together with access to all the materials used and ongoing collaboration and networking tools. This allows them to keep up to date with the latest Care Coordinator and Social Prescribing developments and to have their say within the Care Coordinator community.

In addition to the telephone support, delegates have access to on-demand mentoring for 12 months to suport them in developing their career as a Care Coordinator.

Delegates also have the opportunity to join a Peer Support Network with colleagues on the course. This builds lasting personal and professional relationships, and some of our Peer Support Networks have now been in place for several years.
