STEP 1
� Complete the Pre-Application Registration Form

STEP 2
� Submit the following requirements ONSITE in a long brown envelope:
• Accomplished Application Form

• Photocopy of Transcript of Records (TOR)
• Two (2) 2×2 ID photos
• Two (2) sealed Recommendation Forms from former professors and/or immediate supervisors

(Each recommendation must be placed in a sealed white mailing envelope)
STEP 3
� Pay the Processing Fee: PHP 250.00 at the CASH Office
� Wait for official confirmation of eligibility to take the MEET
STEP 4
� Pay the Testing Fee: PHP 600.00 at the CASH Office
STEP 5
� Claim your Test Permit at the MEd Office.
