I authorize The GO Church, Chicago, its assigns, and transferees to copyright, use, and publish the same in print and/or electronically.

I agree that The GO Church, Chicago may use such photographs and videos of me with or without my name and for any lawful purpose, including, for example, such purposes as publicity, illustration, advertising, and web content.

I understand that:

• The photographs and videos will be used for promotional purposes including social media, website content, and marketing materials.
• I will not receive any form of compensation for the use of these photographs and videos.
• I release The GO Church, Chicago from any claims, demands, and liabilities related to the use of the images.
