• Our clinic is fully ADA compliant and wheelchair friendly.
• Please bring your DD214 and your photo I.D. to your appointment. DD214s will be requested on site to prove veteran eligibility. An electronic copy of the DD214 will be accepted with photo I.D.

• Appointments are required. We will process requests for all appointments online on a first come first served basis.
