To educate and promote best practice, linked to Long term condition management and frailty.

Networking and celebration of best practice and innovation happening within general practice.

9:15 – 10:15 An Introduction to Cardio-renal-metabolic disease with Paul Rushton

• Illustrate a guideline-based approach to cardio-renal-metabolic (CRM) diseases to facilitate early diagnosis and management.
• Understand the interconnectedness of the CRM conditions.
• Understand the incidence and prevalence of CRM conditions and the impact of having one or more of these conditions.
• Examine cases that highlight complexities in CRM disease.
