End-of-life care for a loved one with intellectual and developmental disabilities (IDD) is a sensitive and often challenging topic for families. This session will provide the tools and knowledge you need to navigate this journey with confidence including introducing key terms and language related to end-of-life care (e.g., MOLST, 6-Step Process, Health Care Proxy), exploring how you can support yourself and your family through this process, and emphasizing the significance of including your loved one in decision-making as much as possible, empowering them to be part of this critical conversation. We’ll also explore the availability of services in group home settings and what families can expect in such environments.
The Third Age Committee of New York State (NYS) is composed of health and human service professionals from across New York, committed to advocating for and spreading awareness of the needs of older adults with intellectual and developmental disabilities (I/DD).
Our mission is to plan, develop, and promote educational forums and additional resources for learning and discussion of aging and I/DD among professionals, clinicians, self-advocates, families, and caregivers of older adults with I/DD. These resources will allow participants to create, update and provide valued opportunities for older adults with I/DD.
Our vision is to be the premier educational resource for people to gain new information about aging and (I/DD) from experts across the country.
