About this Event

The Health Inequalities Community of Practice is a regular forum for people working or volunteering on the 'frontline' in health, social care and support organisations who have a common interest in supporting people in Aylesbury experiencing health inequalities.

The Community of Practice is a safe space for those working directly with people in the Aylesbury area to learn from each other, share knowledge, ask questions and explore solutions to benefit local people based in day-to-day practice.

Anyone who works or volunteers in the Voluntary, Community & Social Enterprise sector (VCSE), primary care, mental health services, adult social care, or local authority, with a common interest in supporting people’s health and wellbeing and reducing health inequalities, is welcome.

This session is in addition to the usual thematic sessions and is open to members of both the Aylesbury and High Wycombe Communities of Practice.

It will focus on Integrated Neighbourhood Teams (INTs) and collaborative partnerships.

INT working brings together the NHS (GPs, community health), Social Care, Local Authorities (housing, police, fire, education) and Voluntary, Community & Social Enterprises (VCSEs) – to provide holistic, proactive care, focusing on holistic health and wellbeing by integrating services beyond traditional healthcare settings and to foster joined-up, personalised support for residents. INTs are central to improving local outcomes and implementing the NHS 10-year plan for more integrated, community-based care. We encourage you to attend, share your experiences, and help shape the future of care in Buckinghamshire.

The workshop will be led by the Integration Leads from Aylesbury and Wycombe, Nellie Reynolds and Dee Irvin.
