Come meet a representative from NY Connects at the Sheepshead Bay Library on April 1st and April 22nd from 12pm to 2pm.
NY Connects is New York State’s Aging and Disability Resource Center program. Our mission is to help older adults and people with disabilities maintain independence in their homes and communities by providing information, referral, and assistance with applying for benefits and programs they may be eligible for. Through person-centered counseling, we connect individuals to services such as Medicaid, SNAP, housing supports, long-term care services, transportation, and other community resources that support independent living.
