As the Aged Care Bill/Act 2024 introduces substantial shifts in how care is managed and delivered, it is critical for home care providers to be proactive.
With the introduction of the Support at Home program, the role of care managers is undergoing transformation. Care management will become a crucial component of the proposed Clinical Care and Specialised Supports category, bringing a wider scope of responsibilities. Care managers will not only focus on traditional care coordination but will also take on more dynamic roles in clinical and non-clinical oversight.

This workshop will focus on the evolving role of care managers in light of home care reforms, emphasising how these changes will impact care management delivery. Participants will explore key aspects of care management, including the expanded scope of practice, effective partnering with consumers, enhanced care coordination, integration with clinical care, and conducting both welfare and health check-ins.
The workshop will also examine how future care management will differ from current approaches, offering insights into the new responsibilities and expectations under the Aged Care Bill/Act 2024. Participants will leave with a clear understanding of what is needed to ensure their care management practices remain fit for purpose, aligned with best practices, and adaptable to both current and future home care environments.

- Recognise the future care manager role and care manager capability requirements.
