Changing Role of the Care Manager
About this Event

🌟 Why is understanding the evolving role of care managers crucial in 2025?

🌟 Are your front-line workforce members prepared for the aged care reform changes?

🌟 Have you explored care management scenarios to plan and assess your care model for the future?

As the Aged Care Bill/Act 2024 introduces substantial shifts in how care is managed and delivered, it is critical for home care providers to be proactive.

With the introduction of the Support at Home program, the role of care managers is undergoing transformation. Care management will become a crucial component of the proposed Clinical Care and Specialised Supports category, bringing a wider scope of responsibilities. Care managers will not only focus on traditional care coordination but will also take on more dynamic roles in clinical and non-clinical oversight.

As a provider these changes present both challenges and opportunities. Being well-prepared means not only understanding the upcoming reforms but also equipping your care managers with the skills and knowledge to thrive in their expanded roles. This workshop will:

  • Help you interpret the new regulations and understand how they apply to your role as a care manager.
  • Provide scenario-based planning so you can anticipate and manage changes to care coordination, clinical oversight, and service delivery.
  • Offer insights into staff development, ensuring you and your team are equipped to meet the expanding responsibilities and demands of care management.
  • Highlight opportunities for professional growth and innovation, positioning you to excel and lead in the evolving landscape under the new Aged Care Bill/Act.

At LPA, we’re here to help you navigate the changes and emerge stronger in this new era of home care. By understanding and preparing for the evolving role of care managers, your organisation can remain compliant, competitive, and ready to provide the best possible care to your clients.

🔥 Overview

This workshop will focus on the evolving role of care managers in light of home care reforms, emphasising how these changes will impact care management delivery. Participants will explore key aspects of care management, including the expanded scope of practice, effective partnering with consumers, enhanced care coordination, integration with clinical care, and conducting both welfare and health check-ins.

The workshop will also examine how future care management will differ from current approaches, offering insights into the new responsibilities and expectations under the Aged Care Bill/Act 2024. Participants will leave with a clear understanding of what is needed to ensure their care management practices remain fit for purpose, aligned with best practices, and adaptable to both current and future home care environments.

🚩 Learning Outcomes

By the end of this workshop you will be able to:

  • Understand how the home care reform environment is impacting the provision of care management.

When it comes to online training, we know that staring into the “virtual classroom” can sometimes miss those moments that face-to-face events deliver. So, we are mixing things up. In a time of transformation in the sector, we want to make sure now, more than ever, that you are being supported. We will be focusing even more on interactive, immersive training sessions (think break-out rooms, cameras on (yes scary), opportunities to network, and of course maintaining the LPA way of tangible tools and resources).
