About this Event

Community Based Organizations (CBOs), in a "do more with less" environment, are navigating the challenge of increasing service demands associated with holistic person-centric  program expectations. 

The shift emphasizes improved equitable access, client navigation, and operationalized collaboration between human and social care organizations and healthcare providers.

This is an introduction to the technology-enabled network approach, a key element of the new emerging Community Care Hub model.


November, 2023 - Health and Human Services issued a with the intent of catalyzing efforts at the community-level to encourage partnerships across sectors, including health care, social services, public and environmental health, government, and health information technology to address social needs with the goal of improving the health and well-being of every American.
