Are your care plans so meaningful that content is read at a birthday party or included ina memorial service? Care Plans have the potential to support relationships andlegacy. However, the reality is that person-directed care plans are often reduced tousing the pronoun “I” and individualizing content by adding preferences, goals andpriorities. The elder’s voice and their story are usually lost. Itis possible to meetregulations and create meaningful care plans to create stories worth telling.
This education uses the Eden Growth Model, Six Steps to Lasting Change. Participants will be asked to bring one “challenging” elder’s care plan to the educationalong with their community care plan policy. A “challenging” elder is someone whose choices are considered risky because recommendations are not followed.
A policy template and risk evaluation format will be provided to support participants inthis work.
