Meeting on 11 July 2022, 2pm start (arrivals from 1:30)

You may already be familiar with investigating incidents, this session will highlight some of the common pitfalls when investigating incidents. Root cause analysis tools are useful but as with any tool they can be misapplied. The speakers at this event both worked as Inspectors for the Health and Safety Executive and have conducted many investigations as well as critically reviewing other’s investigations.

About the speakers:

Sarah Stranks

Sarah is a Health, Safety & Environmental professional with over 15 years’ experience in both the public and private sectors. With a background in environmental science (specialising in acoustics and vibration). Sarah joined HSE as an Inspector in 2010, gaining experience of both the Scottish and England & Wales legal systems. Sarah undertook inspections in a wide variety of industries and conducted investigations into workplace accidents including fatalities; cases of ill health; and dangerous occurrences. She is skilled in gathering and managing evidence, decision making, taking appropriate enforcement measures, writing concise legal investigation reports, preparing prosecution files, and presenting prosecution cases in Magistrates’ Court.

Sarah has worked in Tier 1 construction and a fast-growing environmental consultancy to raise H&S standards. She has now set up her own consultancy to support a range of companies and deliver relevant HS&E training. Sarah is also a tutor at the University of Strathclyde on their 3-year postgraduate programme (distance learning) – Safety and Risk Management (PgCert/PgDip/MSc).

Nick Patience

Nick has a wide range of professional experience starting as a Registered Nurse working in the fields of Trauma, Occupational Health and Intensive Care. He was an Inspector with HSE for over 20 years and has investigated a wide range of serious incidents in various settings including: construction, healthcare, manufacturing, and the utilities sectors. He has presented cases in Magistrates’ Court, supported prosecutions in Crown Court and has given evidence at Inquests.

Since leaving HSE he has set up his own consultancy and is a Mental Health First Aid Instructor and OSHCR registered consultant. He delivers training and consultancy for a range of clients. He specialises in providing Investigation (including Root Cause), Interviewing and Mental Health training.
