The Arts Consortium is partnering with the Tulare Kings Hispanic Chamber of Commerce to have 4 artists at their Dia de los Muertos event.

Date: Friday, November 1, 2024
Location: Church Street; between Main St. & Center Ave. (Downtown Visalia)
Time: 5:30pm - 10:00pm
This registration form will help us reserve the 4 booth spots. A waitlist option will be available. Having a waitlist registration ticket does not guarentee your participation in this event. The waitlist will serve as a list of artists the Arts Consortium can reach out to in case a registered artists needs to drop out.
ALL COMMUNICATION WILL BE DONE THROUGH THE EMAIL YOU PROVIDE IN THIS REGISTRATION FORM PLEASE CHECK YOUR EMAIL REGULARLY.
There are a series of questions that you need to answer after you click the Register button; these will be available after, in a new screen. Please make sure you answer these questions.
Requirements
- You must be an Arts Consortium Annual Membership Program holder. If you are not a member please sign up for the membership program before October 18, 2024 @ 12:00pm.
- Participants will be responsible for providing their own canopy, table, and chairs.
- Booth spot is about 10' x 10' area and are assigned on a first-come, first served basis.
- All art must be original and family friendly.
- Set up is from 1:00pm - 4:00pm.
- Participants can take down their booth no earlier than 8 p.m.
- No beverages may be sold unless agreed upon by the TKHCC (event organizer) and arranged in advance.
- No TV, radio, or amplified music allowed.
- BUSINESS INSURANCE: If you can provide proof of business insurance as part of the TKHCC's requirements to participate in this event, then please send that to [email protected] as soon as you finish submitting this application. If you cannot provide this, then the Arts Consortium will provide one-time insurance coverage to you for this event - So long as you abide by all of the aforementioned requirements and so long as you do not violate the Arts Consortium's Standards of Conduct (included below) in any way.
Notice Regarding Photography/Audio/and Video Recordings
Tulare Kings Hispanic Chamber of Commerce (TKHCC) captures photographs and/or audio/video recordings during events. Such photographs and recordings may be used in TKHCC advertising materials and social media relations to let others know of our events. In addition, local news organizations may hear of our events and TKHCC may allow them to record events for news reporting on special interest features.

ARTS CONSORTIUM ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN ANY/ALL ACTIVITIES ASSOCIATED WITH ARTS CONSORTIUM’S FIRST FRIDAY EVENT AND TULARE KINGS HISPANIC CHAMBER OF COMMERCE EVENT. This includes, by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the personsor entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that there are no health-related reasons or problems which preclude my participation in this activity.
I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity in which I may participate, and that it will govern my actions and responsibilities at said activity.
I hereby take action for myself as follows:
(A) I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for personal injury, property damage, or property theft, THE FOLLOWING ENTITIES OR PERSON:
a. The Arts Consortium, P.O. Box 2696, Visalia, CA 93279 and/or their officers, and the activity holders, sponsors, and volunteers;
b. The City of Visalia, 707 W. Acequia Ave., Visalia, CA 93291 and/or their officers, and the activity holders, sponsors, and volunteers;
c. The Tulare-Kings Hispanic Chamber of Commerce and/or their officers, and the activity holders, sponsors, and volunteers;
(B) INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of released or otherwise.
I acknowledge that the Arts Consortium and the City of Visalia and their officers, or volunteers, are NOT responsible for the errors, omissions, acts, or failures to act of any party, entity, or partner for a specific activity.
Furthermore, I agree to abide by the Arts Consortium’s Standards of Conduct.
I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assignees.
The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
THE VISALIA ARTS CONSORTIUM, INC. STANDARDS OF CONDUCT
The Arts Consortium is a community-based non-profit organization dedicated to connecting the arts,culture, and community. Participation in the organization’s programs as a board member, committee member, event volunteer, office volunteer, staff, artist, or other kind of volunteer is subject to the observance of the organization’s rules, policies, and procedures. The Arts Consortium is an organization of high moral and ethical standards. The activities or behavior described below are inappropriate in view of those moral and ethical standards and are therefore strictly prohibited. Any participant or board, committee, or staff member, volunteer, participating artist, or member who violates this code is subject to sanctions, including removal from the organization.
No participating artist or employee shall:
a. Use abusive language or show discourtesy or rudeness to staff, volunteers, or other participants.
b. Use or possess illegal drugs during the ARTS CONSORTIUM programs or attend programs while intoxicated.
c. Show any type of harassment, including verbal, physical, or visual harassment toward staff, volunteers, or other participants.
d. Show actual or threatened violence toward any individual or group.
e. Conduct themselves in a manner that endangers the life, safety, health, or well-being of others.
f. Bully or take unfair advantage of any participants.
g. Fail to cooperate with any board member, staff member, committee leader, or other official of the organization.
h. Fail to respect the cultures, beliefs, opinions, and decision of others.
i. Fail to treat others with courtesy, sensitivity, tact, and consideration.
j. Authorize the use of or use for the benefit or advantage of any person, the name, logo, endorsement, services, or property of the ARTS CONSORTIUM, except in conformance with the ARTS CONSORTIUM policy.
k. Accept or seek on behalf of any person, any financial advantage or gain of other than nominal value offered as a result of the volunteer or employee’s affiliation with the ARTS CONSORTIUM.
l. Publicly use any ARTS CONSORTIUM affiliation in connection with the promotion of partisan politics, religious matters, or positions on any issue not in conformity with the official position of the ARTS CONSORTIUM.
m. Disclose or use any confidential ARTS CONSORTIUM information that is available solely because of the volunteer’s or employee’s affiliation with the ARTS CONSORTIUM to any person not authorized to receive such information or use to the disadvantage of the ARTS CONSORTIUM any such confidential information, without the expressed authorization of THE ARTS CONSORTIUM.
n. Knowingly take any action or make any statement intended to influence the conduct of the ARTS CONSORTIUM in such a way as to confer any financial benefit on any person, corporation, or entity in which the individual has a significant interest or affiliation.
o. Operate or act in any manner that is contrary to the best interests of the ARTS CONSORTIUM.
p. Operate or act in a manner that creates a conflict with the interests of the ARTS CONSORTIUM and any organization in which the individual has a personal, business, or financial interest.
The individual shall disclose such conflict of interest to the ARTS CONSORTIUM President of the governing board, the Executive Director, or the Board Treasurer, as applicable, upon becoming aware of the conflict of interest. Where required, the individual shall absent him or herself during deliberations, and shall refrain from participating in any decisions or voting in connection with the matter.
I have read and understand the Arts Consortium Standards of Conduct. I agree to abide by the rules described above and understand that I may be removed as a member or other participant if I violate any of the above stated rules.
ARTS ARTSORTIUM MEDIA RELEASE
You understand while participating in this activity, you may be photographed. You agree to allow your photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns.
You hereby grant permission to Visalia Arts Consortium, Inc.,hereinafter known as the “Media” to use your image (photographs and/or video) for use in Media publications.
You hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to you or unknown, and you waive any right to royalties or other compensation arising from or related to the use of the image.
You fully understand the contents, meaning and impact of this release. You understand that you are free to address any specific questions regarding this release by submitting those questions in writing prior to registering, and you agree that your failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.
--- Failure to comply with any of these conditions will constitute as grounds for being removed from future Arts Consortium events.
