Join the Health Care Consumers’ Association with Palliative Care ACT and Canberra Health Services for our Advance Care Planning Week Afternoon Tea! Advance Care Planning Week is an opportunity to understand and document your choices for health care and end-of-life care if there comes a time when you can’t make your own decisions. This year’s theme “Your story, your choice” reminds us that everyone’s story is unique and it is important to know and make your own choices for the future.

How it will run
You will join a table of 6-7 other people. Presenters will join your table for a 15-minute conversation and questions about their area of expertise. The presenters will then rotate to the next table so you can hear about each topic.
Topics will include:
Palliative care support and services - Palliative Care ACT
Advance Care Planning - Canberra Health Services
Finding the right aged care - Community Options’ Care Finders
Aged care and finances - Services Australia
Planning ahead with cancer - Cancer Council ACT
