Course Description:
It is the purpose of this course to provide a systematic approach to diagnosis and treatment planning the complex interdisciplinary dental patient with a common language that may be used by the orthodontist, periodontist, and oral and maxillofacial surgeon, as well as the restorative dentist.
The four Global Diagnoses which dictate all interdisciplinary treatment planning will be defined. A set of questions will then be presented which will aid the interdisciplinary team in the diagnosis and treatment planning of the complex dental patient.
The complex interdisciplinary patient will require treatment which incorporates an in-depth knowledge of smile design, proper occlusion, and the ability to work with a team, to include the dental laboratory technician. Commonly this patient requires altering the Vertical Dimension of Occlusion (VDO) which adds complexity and cost.
Porcelain crowns or veneers are the typical mainstay of this type of treatment. However, utilizing composite veneers by injection molding, 3D printed posterior onlays, and shell provisional crowns can offer a less costly option. These materials can also be used as an interim restorative material while phasing treatment and allowing the patient to proceed through treatment at their own pace. With the advent of new digital technology, it is now possible to utilize flowable composite, 3D printed or milled transitional restorations to stabilize a deteriorating dentition.
These restorations can also be used for pre-orthodontic or intermediate orthodontic bonding which significantly improves communication between the orthodontist and the restorative dentist. In a patient receiving pre-restorative orthodontic treatment, any tooth which has lost natural anatomic contours due to attrition, erosion, or caries, should receive pre- or intermediate orthodontic bonding to direct the orthodontic treatment. This makes orthodontic treatment more efficient and precise and allows the restorative dentist to place more conservative definitive restorations at the completion of orthodontics.
