The theme of the 4th Annual Health Care Transition Summit is Connection: Humanizing the Health Care Transition from Pediatric to Adult Serving Systems. We will bring together a statewide collaboration of advocacy, clinical, education, and policymaking experts to share best practices and exchange knowledge that successfully supports youth as they transition to adult services. The Summit will offer participants valuable insights on driving advancement in the field of Health Care Transition (HCT). Participants can use this knowledge to enhance their clinics, hospitals, programs, and overall state practices to support safe transition from pediatric to adult care systems.
We are delighted to welcome Dr. Patience White and Peggy McManus, from Got Transition, a program of the National Alliance to Advance Adolescent Health. Dr. White and Ms. McManus, along with clinical experts from Gillette Children’s, and other esteemed guests, will provide expertise and guidance to foster engaging dialogue centered on current conversations in pediatric to adult HCT.

Cancellation/Refund Policy: Cancellations received by 03/23/26 will be eligible for a full refund. Cancellations received between 03/24/26 and 04/19/26 will be eligible for a 50% refund. Cancellations received on or after 04/20/26 will not be eligible for a refund. All refunds issued will be less any applicable Eventbrite registration fees and will be issued to the original payment method used to register for the Summit.
