• Applicant ID
• Proof of Residency (Electric Bill, Telephone Number, etc.)
• Birth Certificate for Every Child under 12
• Proof of Income such as a Recent Paystub, SSI Benefits, Child Support, SNAP/TANF, Unemployment, etc.
DOCUMENTOS REQUERIDOS PARA LA INSCRIPCIÓN:

• Certificado de Nacimiento Por Cada Niño Menor de 12 Años
