WYICB All Age Integrated Diabetes Summit: A Stock take & Priority Setting
The intention of the Summit is to (subject to refinement):
- Determine our shared vision/the future for diabetes care in West Yorkshire
- Identify how we might get to this future state and over what time period
- Agree the priorities for 2025/26 and beyond which may be a) all age overarching priorities (e.g. back to basics for T&C process attainment; young adult transition care optimisation; diabetes med technology fair access) b) Place specific priorities c) WYAAT / trust specific priorities
- Consider how we might influence/feedback to inform the NHSE/WY 10 year strategy with a diabetes lens
- Identify actions and who is responsible aligned to our collectively agreed priorities.
