Children age 6 months - 18 years, regardless of insurance coverage
No appointments. Walk-ins are welcome.
You will need to fill out a consent form for any individuals receiving vaccine at the event. To save time at the event, you may download, print and complete the demographic portion of the consent form for each individual receiving vaccine at this event and bring it with you. Please use the Child Consent Form for individuals who are under 19 years of age (0-18).
