About this Event

UH LGBTQ+ & Gender Care and University Hospitals Urology Institute are proud to offer, and excited to lead, this educational (non-CME) course on masculinizing genital gender affirmation surgery (bottom surgery) for surgeons interested in offering these services to their gender expansive patients.


Course Description:

A mixed model workshop designed to familiarize participants with masculinizing genitoplasty for gender affirmation and phallic reconstruction after trauma or surgery. The one-and-a-half-day course will consist of presentations, surgical videos, panel discussions and cadaver dissections. Various approaches to staging phalloplasty and common phalloplasty approaches will be discussed – including radial artery phalloplasty, abdominal phalloplasty and anterolateral phalloplasty. Metoidioplasty as an option for patients not electing phalloplasty will be discussed – including the two most common metoidioplasty techniques: Belgrade and ring metoidioplasty. Urethral reconstruction as part of masculinizing genitoplasty (primary and reparative) will be addressed.


Background:

Interest in gender affirmation surgery has increased manifold in the last decade due to enhanced demand, insurance coverage, academic involvement and public acceptance. Genital gender affirmation surgery (bottom surgery) geared toward masculinization (masculinizing bottom surgery) is generally considered to be challenging due to the complexity of tissue transfer techniques and potential for complications, thus is offered in very select institutions. This course will acquaint participants with a detailed understating of masculinizing bottom surgery, and equip them with the skills to incorporate offerings into their own practice.


Learning Objectives:
  • Define the various types of masculinizing genital surgeries and their indications, advantages, outcomes and complications
  • Explain the staging techniques for phalloplasty including single stage, phalloplasty-first and ‘meta’-first staging
  • Demonstrate the anatomy and dissection of metoidioplasty, radial artery phalloplasty, abdominal phalloplasty, and anterolateral thigh phalloplasty
  • Evaluate the suitability of patients for various masculinizing genital surgeries

Select live dissections will be performed with the opportunity for participants to perform dissections independently. Remaining stations will have pre-dissected cadavers demonstrating techniques of interest.

  • Station 1: Radial Forearm – Flap Elevation
  • Station 2: Recipient Vessels, Nerves
  • Station 3: Vaginectomy, Scrotoplasty, Urethral Join-Up & Clitoral Transposition
  • Station 4: Metoidioplasty – Ring Type
  • Station 5: Abdominal Phalloplasty
  • Station 6: ALT Phalloplasty

Panel discussion and boxed lunch will follow the Cadaver Lab.
